Why does it show a lower number than the card's advertised rate?

Because advertised rates ignore monthly caps, excluded categories and surcharges. A 5% rate capped at ₹500 a month is 5% only up to ₹10,000 of spend, and less on every rupee after that. Reward points are also valued at a conservative redemption rate rather than the issuer's best-case example.

Does the ranking account for the annual fee?

The monthly figure is reward return only, with the annual fee shown alongside it. Subtract the fee plus GST from the annual total unless your total spend clears the waiver threshold.
